For Students Requesting Disability-Related Accommodations


 

1.  Register with the CSD

Register online through MyAccess, the confidential and secure portal for the Center for Students with Disabilities.  If preferred, you can also mail a completed registration form to:

2. Submit Documentation

Review the CSD's documentation guidelines with the treatment provider who can best describe the nature, severity, and impact of the condition(s).  Submit documentation that meets these guidelines, establishes your disability, and addresses the need for accommodations.  Update your documentation whenever there are changes or requests for clarification.  For students who are d/Deaf or hard of hearing, documentation should include the following:

Type, degree and configuration of hearing loss, including frequency and intensity;

Auxiliary aids or services currently used; and

Specific  type, brand, model, color, and attachments (e.g. streamers, receivers) for personal hearing device(s).

3. Meet with your DSP

After registering, you will be assigned a Disability Service Professional (DSP), usually a member of the UCIS Team.  Your DSP will work with you to determine appropriate accommodations tailored to your needs.  Schedule a meeting with your DSP as soon as possible to discuss the accommodations process.

4. Request UCIS Services (as needed)

Continue to request sign language interpreters and/or live captioning as needed using our online form.  Please make sure to fill out the form completely and accurately so that we can coordinate services with all responsible parties.

UCIS will make every effort to accommodate requests at any time, for any UConn-sponsored event, activity, or program.  However, we recommend the following timeline to facilitate coordination of services:

For large-scale, recurring, or public events, submit requests 3 weeks in advance.

For classes, submit requests upon registration or at least 3 weeks in advance.

All other requests should be submitted at least 3 business days in advance.

Requests received outside the recommended timelines may be difficult to accommodate with preferred services and/or providers. In such cases, UCIS will work with you to devise appropriate alternative arrangements, as needed. If you need to change or cancel any requests, please notify UCIS as soon as possible at ucis@uconn.edu.
