For Faculty & Staff Requesting Disability-Related Accommodations
1. Contact the HR ADA Accommodations Case Manager.
UConn's Department of Human Resources (HR) is charged with processing and facilitating requests for workplace accommodations for employees. Employees seeking an accommodation, or supervisors or managers who have been presented with a request for an accommodation, should contact the ADA Accommodations Case Manager, Vicki Fry, at 860-486-2036 or email@example.com to request accommodations.

2. Request UCIS services as needed.
Continue to request sign language interpreters and/or live captioning as needed using our online form. Please make sure to fill out the form completely and accurately so that we can coordinate services with all responsible parties.
UCIS will make every effort to accommodate requests at any time, for any UConn-sponsored event, activity, or program. However, we recommend the following timeline to facilitate coordination of services:
For large-scale, recurring, or public events, submit requests 3 weeks in advance.
All other requests should be submitted at least 3 business days in advance.
Requests received outside the recommended timelines may be difficult to accommodate with preferred services and/or providers. In such cases, UCIS will work with you to devise appropriate alternative arrangements, as needed. If you need to change or cancel any requests, please notify UCIS as soon as possible at firstname.lastname@example.org.
